Summer is here and, for athletes, that often means it's time to get sports physicals scheduled. Incoming 6th graders need a sports physical dated after January 1st, 2023. Click here for the Issaquah School District Physical Form. 

Sports registration for middle and high school students is done through FinalForms. New 6th grade families will receive an invitation this summer to create a FinalForms account. For families of current Pacific Cascade athletes and for incoming 6th graders who have older siblings, parents can use their existing login information. Once the sports physical form is signed by your physician, families can upload it into their student's FinalForms account.

Physicals are good for two years if your student is not new to middle or high school, so many 7th and 8th grade athletes won't need new physicals. Check your student's FinalForms account to see if your student's physical is current for the next school year.
